Offline Payment plugin, for bank transaction from donor

Thanks its works now, just two last questions. I used the {donate_id}, {donor_name} and {donation_amount} variables but they do not show the entry/fill out (amount, ID or donor name) just this:

Is it possible to have two templates anwser one for offline payment and a diffrent one if payed over paypal?
